인지적 병식 척도(Beck's Cognitive Insight Scale)의 표준화 예비 연구

Abstract

본 연구는 정신분열병 환자에게서 특징적인 인지적 병식 부족을 평가하기 위하여 고안된 Beck의 인지적 병식 척도(Beck's Cognitive Insight Scale; 이하 BCIS라 함)를 한국의 정신분열병 환자를 대상으로 평가하기 위해 한국판 BCIS를 제작하고, 이것의 신뢰도와 타당도를 검증해 봄으로써, 표준화된 검사로 발전 시킬 수 있는 기초자료를 얻는데 있었다. 또한, 기존의 임상적 병식 척도가 실행기능과 정신분열병의 부적 증상과 관련된다는 선행연구에 뒷받침해, 개발된 인지적 병식 척도가 어떤 인지기능 및 증상과 관련되는지 조사해 보고자 하였다. 즉, 본 연구의 문제는 다음과 같다. 연구문제1. BCIS를 국내의 정신분열병 환자를 대상으로 실시 할 수 있도록, 한국어판 BCIS를 제작한다. 연구문제2. 한국판 BCIS 척도의 신뢰도와 타당도를 검증해 본다. 연구문제3. 개발한 인지적 병식척도인 BCIS와 기존의 임상적 병식척도인 SUMD가 각각 어떤 신경심리 기능 사이의 관련성이있는지 조사해 본다. 연구문제4. BCIS와 SUMD와 PANSS와의 관련성을 조사한다. 연구 대상은 경기 지역 소재의 정신병원에서 정신분열병으로 진단 받은 환자들 50명을 대상으로, 병식을 측정하기 위해 BCIS, SUMD, ITAQ, 신경인지기능을 측정하기 위해 신경심리검사 바테리, 정신분열병 환자의 증상을 측정하기 위해 PANSS를 실시하였다. 본 연구 결과는 한국판 BCIS 척도가 신뢰롭고 타당하며, 기존의 임상적 병식 척도와 달리 새로운 측면인 인지적 병식을 측정한다는 것과 Beck 등이 가정한 2요인 구조가 한국판 BCIS 척도에서도 타당하다는 것을 뒷받침 해 주었다. 정신분열병 환자의 인지적 병식 부족은 실행기능의 특정한 측면과 관련이 있었다. 또한, 정신분열병 환자의 인지적 병식 부족은 정신분열병 환자의 양적 증상 중 망상, 부적 증상 중 상동적 사고, 일반 정신 병리의 상동 행동, 병식 부족, 비협조성, 몰입행동과 관련이 있었다.|The purposes of this study were to develop a Korean version of Beck Cognitive Insight Scale(Beck et al., 2004), and to examine the reliability and validity of it. In addition, this study investigated the relationships between cognitive insight and clinical insight, and between insight, neuropsychological function and symptoms of schizophrenia. Participants were 50 inpatients and outpatients with schizophrenia who completed a battery of measures consisting of insight scales, neuropsychological tests and PANSS. The results indicate that a Korean version of the Beck Cognitive Insight Scale(BCIS) is a reliable and valid instrument for the assessment of cognitive insight. The Self-reflectiveness of BCIS correlated with specific aspect of executive function of schizophrenia. There were significant associations between total score of BCIS and delusion, stereotyped thinking, general psychopathology and total score of the PANSS.
