논 문 개 요

본 연구는 음악중심의 심리치료가 정신과 폐쇄병동에서 생활하는 정신장애 환자의 신체화와 스트레스에 미치는 영향을 알아보기 위해 실시되었다.
연구대상자는 천안시 D구 H병원, 인천시 N구 J병원에서 조현병, 기분장애 진단을 받고 보호자 동의에 의해 폐쇄병동에 입원된 30∼60세 사이, 신체화와 스트레스 증상이 나타나는 정신장애 환자로 실험집단 5명, 통제집단 5명, 총 10명이 선정되었다. 본 프로그램은 2013년 4월 1일부터 2013년 4월 30일까지 실험집단의 대상자에게 주 2회, 60분, 총 8회기가 실시되었고, 신체화와 스트레스 증상의 검사도구로는 간이정신진단검사와 한국어판 스트레스 척도가 사용되었다.
연구 설계는 사전-사후 통제집단설계이며, 각 회기 음악 감상, 악기 연주, 마인드 맵, 음악과 심상(Music & Imagery)으로 구성된 음악중심의 심리치료 프로그램이 실시되었다. 음악중심의 심리치료는 내담자와의 관계형성 및 긍정적 자아 탐색의 도입단계, 유년기부터 성인기까지의 깊은 내면 탐색 및 표출의 전개단계, 내적 문제 확인 및 자기 지지의 마무리단계로 진행되었다.
본 연구 결과는 첫째, 음악중심의 심리치료 프로그램이 실시된 실험집단은 통제집단과 비교하여 사전-사후 신체화 점수가 감소하여 통계적으로 유의미하였다(p<.05). 둘째, 음악중심의 심리치료 프로그램을 경험한 실험집단은 통제집단과 비교하여 사전-사후 스트레스 점수 분석 결과, 통계적으로 유의미한 차이를 보이지 않았지만(p>.05), 스트레스 감소 경향성이 확인 되었다(p=.092). 셋째, 실험집단과 통제집단의 신체화와 스트레스 점수는 스트레스 점수가 높아질수록 신체화 점수가 증가하는 정적 상관관계가 나타났다(r=.547).
본 연구를 통해 음악중심의 심리치료는 폐쇄병동에서 생활하는 정신장애 환자의 신체화와 스트레스 증상을 감소시킬 수 있는 효과적인 치료 중재가 될 수 있음이 확인될 수 있었다.|The purpose of this study is to examine the effect of music-centered music therapy program which is constructed by pretest-posttest group design on the mental disorder’s somatization and stress.
This study selected 5 hospitalization patients(2 men & 3 women) who was diagnosed as mental disorder(Schizophrenia, Mood disorder) for experiment group in H-hospital D-gu, Cheon-an between April 1. 2013 and April 27. So 5 hospitalization patients(5 women) who was diagnosed as mental disorder(Schizophrenia, Mood disorder) for control group in J-hospital N-gu, In-cheon between April 3. 2013 and April 30.
The musical program was composed of music listening, instrument playing, mind map and Music & Imagery(MI). Especially MI was practiced to patients as only support level for considering patients’ stabilization and therapist’s step level. Each time for therapy was proceeded for 60 minutes twice per week and total session was 8 times.
The result of the study shows as below.
First, music-centered music therapy has effect on the mental disorder’s somatization between experiment and control group that was confirmed significant meaning(p<.05)
Second, music-centered music therapy has not effect on the mental disorder’s stress between experiment and control group that was not confirmed significant meaning(p>.05). but tendency of reducing-stress was considered(p=.092).
Third, somatization and stress, as somatization level increases, stress level also increases, which means there is significant positive relationship between somatization and stress(r=.547).
As a result, through this study, it was proved that the Effect of Music-centered music therapy Program on the Mental Disorder’s Somatization. Also through tendency of reducing-stress, Music-centered music therapy Program could be considered as therapy intervention about stress.
