PTT 모델을 근거로 한 보건소 방문간호사의 문화적 인식, 문화적 지식, 문화적 수용성 및 문화 역량과 영향요인 연구

Abstract

본 연구는 PTT(Papadopoulos, Tilki & Taylor) 모델을 이론적 기반으로 지역사회 방문간호사의 문화적 인식, 문화적 지식, 문화적 수용성 및 문화 역량을 파악하고 영향요인을 확인함으로써 지역사회 방문간호사의 문화 역량을 향상하고 프로그램 개발에 기초자료를 제공하기 위하여 연구를 진행하였다. 연구대상은 S시 25개 보건소에 근무하는 방문간호사 250명으로, 2020년 8월 10일부터 9월 30일까지 구조화된 설문지를 이용하여 방문간호사의 문화적 인식, 문화적 지식, 문화적 수용성 및 문화 역량을 측정하였다. 연구 도구는 총 55문항으로 일반적 특성 7문항 성별, 나이, 결혼 여부, 종교, 근무 기간, 가족 동거 여부, 교육수준, 문화 관련 특성 6문항 다문화 관련 교육 여부, 외국어 교육 여부, 외국어 대화 수준, 외국인 접촉기회, 외국인 환자 접촉 기회, 해외 체류 경험 과 문화적 인식 6문항, 문화적 지식 10문항, 문화적 수용성 12문항, 문화 역량 14문항으로 이루어졌다. 수집한 자료는 연구목적 및 변수의 특성에 따라 실수, 백분율, 평균, 표준편차, t-test, ANOVA, Pearson 상관계수, 다중회귀 분석으로 분석하였다.

본 연구의 결과는 다음과 같다. 첫째, 대상자의 문화적 인식, 문화적 지식, 문화적 수용성, 문화역량 평균은 각각 5점 만점에 문화적 인식 2.74점, 문화적 지식 2.28점, 문화적 수용성 2.53점, 문화 역량 3.26점이었고, 모든 문항의 평균은 2.70점으로 확인되었다. 둘째, 방문간호사의 문화적 인식, 문화적 지식, 문화적 수용성 및 문화 역량의 상관관계로 방문간호사의 문화적 인식은 문화적 지식(r=.555, p<.001), 문화적 수용성(r=. 524, p<.001), 문화 역량(r=. 499, p<.001)과 유의한 상관관계를 보였다. 문화적 지식은 문화적 수용성(r=. 663, p<.001), 문화 역량(r=. 434, p<.001)과 유의한 상관관계가 있는 것으로 나타났으며 문화적 수용성은 문화 역량(r=. 627, p<.001)과 유의한 상관관계가 있는 것으로 나타났다. 셋째, PTT 모델을 근거로 방문간호사의 문화적 인식, 문화적 지식, 문화적 수용성, 문화 역량에 영향을 미치는 요인을 확인한 결과 방문간호사의 문화적 인식에 영향을 미치는 요인은 외국어 대화 수준(β =. 158, p=. 007), 문화 역량(β=. 457, p<.001)이었고, 방문간호사의 문화적 지식에 영향을 미치는 요인으로는 외국어 대화 수준(β=. 190, p=.001), 외국인과 접촉 빈도(β=. 160, p=. 007), 문화적 인식(β=. 458, p<.001) 으로 나타났다. 방문간호사의 문화적 수용성에 영향을 미치는 요인은 문화적 지식(β=. 602, p<.001) 으로 나타났고 방문간호사의 문화 역량에 영향을 미치는 요인은 종교, 가족과의 동거 여부, 문화적 수용성(β=. 599, p<.001)이었다.

본 연구 결과를 기반으로 지역사회 방문간호사의 문화 역량을 함양시키기 위해서는 PTT 모델이 제시한 단계적 요인들을 사정하고 그 결과를 바탕으로 대상자의 상황에 맞는 영향요인을 포함한 프로그램 개발 및 적용이 필요한 것으로 나타났다.|This study conducted a study to improve the cultural competence of community visiting nurses and to provide basic materials for program development by identifying their cultural awareness, cultural knowledge, cultural sensitivity, and cultural competence based on the PTT (Papadopoulos, Tilki & Taylor) model.

The study targets 250 visiting nurses working at 25 health centers in S.C. and measured the cultural awareness, cultural knowledge, cultural sensitivity and cultural competence of visiting nurses using structured questionnaires from August 10 to September 30, 2020. The research tools consist of a total of 55 questions, 7 general characteristics gender, age, marital status, religion, working period, family Cohabitation, education level, 6 cultural characteristics Multicultural education status, foreign language education status, level of foreign language conversation, opportunities for contact with foreigners, opportunity to contact foreign patients, experience of staying abroad and 6 questions of cultural awareness, 10 questions of cultural knowledge, 12 questions of cultural sensitivity, and 14 questions of cultural competence. The collected data were analyzed by a real number, percentage, average, standard deviation, t-test, ANOVA, pearson’s correlation coefficient, multiple regression analysis, depending on the purpose of the study and characteristics of the variables.

The results of this study are as follows: First, the cultural awareness , cultural knowledge, cultural sensitivity , and cultural competence averages of the subjects were 2.74 cultural awareness, 2.28 cultural knowledge, 2.53 cultural sensitivity , and 3.26 cultural competence out of 5 points each, and the average of all questions was identified as 2.70 points.

Second, the cultural awareness of visiting nurses is the correlation between cultural awareness, cultural knowledge, cultural sensitivity, and cultural competence of visiting nurses (r=. 555, p<.001), cultural sensitivity(r=.524, p<.001), cultural competence (r=.499, p<.001) It showed a significant correlation with 001). Third, based on the PTT model, we identified factors that affect the visiting nurse's cultural awareness, cultural knowledge, cultural sensitivity , and cultural competence, and the factors that affect the visiting nurse's cultural awareness are the level of foreign language conversation (β =.158, p=. 007), cultural competence (β=.457, p<.001),and the foreign language conversation level (β=.190, p=.001), influences the cultural knowledge of visiting nurses. frequency of contact with foreigners (β=. 160, p=. 007), Cultural Awareness It turned out to be (β=.458, p<.001), Cultural knowledge (β=.602, p<.001) influences the cultural sensitivity of visiting nurses. The factors influencing the visiting nurse's cultural competence were religion, family Cohabitation status, and cultural sensitivity. It was (β =.599 p<.001)

Based on the results of this study, to foster the cultural competence of community visiting nurses, we assess the step-by-step factors presented by the PTT model and based on the results, we need to develop and apply programs including factors that suit the situation of the target.
